Actor John Amos’s death certificate lists “congestive heart failure” the cause of his death. Amos passed away August 21 at Inglewood Hospital in Los Angeles County, California, and was cremated 9 days later.

An informational copy of the actor’s death certificate was obtained by TMZ. It also confirms that the Amos’s body was cremated on August 30 and that the death had been reported to Los Angeles County by the actor’s son, Kelly Christopher Amos. The certificate also confirms that Kelly Christopher is in possession of the actor’s cremated ashes.

According to TMZ, Kelly Christopher was having a dispute with his sister (and John Amos’s daughter) Shannon. Shannon accused her brother of abusing their father. She also claims that her father died without any family members present. According to Shannon, a woman claiming to be John Amos’s daughter was in the emergency room the night her father died. Shannon also claimed that she believes that woman was Belinda Foster, who had also been claiming to be the actor’s publicist.

A native of the State of New Jersey, John Amos played football for multiple teams in the 1960s. Amos later switched to acting and achieved nationwide fame for his role of the adult Kunta Kinte in the miniseries Roots in 19770 and James Evans Sr. in the television series Good Times between 1974 and 1979. He also appeared on television in The Mary Tyler Show (1970-1977), The West Wing (1999-2006), and The District (2000-2004). He also played supporting roles in a number of movies, including The Beastmaster (1982), Coming to America (1988), Lock Up (1989) and Die Hard 2 (1990).
